I hereby authorize and direct any authorized agents, veterinarians, staff or representatives of Sunny Coast Veterinary to perform the diagnostic and procedures/treatments deemed advisable for my animal(s). I understand that there are certain risks and complications associated with any procedure or treatments, unforeseen conditions may arise that may necessitate the performance of additional procedures.

I also authorize the use of appropriate anesthesia and pain relief medication as needed before, during or after any procedure or treatment needed. I understand that there are risks associated with the use of any medication, especially anesthesia, and I agree to hold Dr. Francheville of Sunny Coast Veterinary and any of her authorized agents, staff, or representatives harmless from any and all liabilities from unforeseen animal behavior that may injure the animal(s) while under anesthesia.
